1. A member who becomes mentally or physically disabled
2. A member who has not reached pensionable age
3. A member has made less than 60 monthly contributions.
4. Invalidity lump sum is only payable once

What should be submitted at the time of making a claim?

1. Recommendation from medical board certifying that the member is incapacitated.
2. Certified medical discharge letter from employer
